​IMPORTANT:

You must bring an original unexpired US Government Issued

Driver's license or Passport.

​

Please bring your Social Security Card if you have it. 

 

If your name differs on your SS card from your Livescan form or driver's license, we must have a copy of your SS card in order to help you correct your identity in the clearinghouse. 

We do the update when we meet with you if you bring your card.​

(The clearinghouse will only correct a name issue with copies of your drivers license and social)

We are there once per month.  On other weeks you will find us at the Sebastian Library between Noon and 1PM.  Book Sebastian Library locatrion if you prefer that option.
