OBTAINING YOUR LIVESCAN REQUEST FORM

If you are a licensed healthcare professional such as doctor, nurse, chiropractor, pharmacist, or any specialty regulated by the Department of Health, you need a live scan form generated from the CHAI or
"Clearinghouse Applicant Initiated" portal.

* Many people are coming for fingerprints that don't need them because they are already eligible in the clearing house.
* Visit chai and check your status before your appointment.
* If you don't have a profile, your fingerprints are expired, or your status says clearinghouse NO, You need new fingerprints.
Go to chai, sign in and you will be sent a one-time use passcode to your email. You must leave the original page open so that you can come back and put in that passcode or it will reset and you'll end up in a loop. If you have trouble leaving a page open while you go to retrieve the email, Go to the website on a desktop and then retrieve the code from your telephone.​

It may say "no profile found". Some people think they're stuck at that page but that is not the case; You simply have to hit initiate screening to move forward.

Many people seem to have trouble if they're on Safari or Firefox, so please use Google Chrome.

On the bottom of your profile page there is an " Initiate Screening" Button.
AFTER INITIATE SCREENING. You either need to enter your personal data or update your personal data then hit next. Proceed thru steps.
If you are on your phone, when you get to the privacy policy, you might have to turn your phone to landscape to acknowledge. The acknowledge button can be hard to find. It's in a blue box with a small check mark. Scroll down and if you still don't see it, hit "Confirm Privacy Policy" at top, or hit the back button and then go forward again and just noodle with it until you can click the check mark, because your results will not be posted unless you affirm the privacy policy.
MOST IMPORTANT ADVICE:
When you get to the end, there's a Live Scan provider search. Don't use it. It takes you away from the site and you usually don't get your form. Ignore the search criteria and scroll past that and you will see "continue without making an appointment". It is just below the search criteria and just above the list of vendors. It's not prominent and does not look like a link, but click that and it will take you right to your live scan form. You do not need to select a provider to get your form, but you need your form to get your fingerprints. Print the Live Scan form or take a screenshot and make sure to capture your screening ID and ORI code. Once you have your form, you can book an appointment here by clicking the link above.
